Coordinated Case Management

Navigating multiple agencies can be overwhelming—but you don’t have to do it alone. At Kids Hub, we use a Multidisciplinary Team (MDT) approach to ensure every child and family receives the most effective, compassionate, and coordinated care possible.

Why Coordinated Care Matters

When allegations of abuse arise, it takes a team of dedicated professionals to respond in a way that protects children, supports families, and promotes healing. Our MDT includes:

  • Law enforcement

  • Child protective services

  • Prosecutors

  • Medical and mental health providers

  • Family and victim advocates

By working together and sharing information, this team approach:

  • Reduces the number of times a child must share their experiences

  • Provides a consistent, trauma-informed response

  • Connects families to needed services and resources

  • Promotes informed, successful outcomes when abuse has occurred

This collaborative model helps minimize further trauma, ensures families don’t fall through the cracks, and supports every step of the healing and justice process.

How the Team Works

The Multidisciplinary Team meets regularly in each county to:

  • Collaborate on active cases through problem-solving and brainstorming

  • Ensure all team members remain informed and up to date

  • Address emerging needs and adjust support plans as necessary

  • Coordinate follow-up services and identify any gaps in care

These meetings are designed to be dynamic and solution-focused, helping the team stay aligned and responsive as each case progresses. Every discussion is grounded in shared goals: Protect the child. Support the family. Strengthen the response.
